DJWH Foundation History

 

The Dr. John Warner Hospital Foundation was founded and incorporated in 1998. The 12-member Board of Directors manages the business and affairs of the corporation. The Foundation is responsible for coordinating fundraising efforts and administering charitable gifts and donations given to the Foundation.

 

Foundation Purpose

 

The purpose of the DJWH Foundation is to foster, promote, support, develop, encourage, maintain, receive, and accept funds for the construction, building, remodeling, support, administration, staff, and any other legitimate purpose or function of supporting the DJWH facility.

For there are few organizations where your financial gift can be used to directly aid your friends, family, and neighbors, where every contribution is vital to providing the best possible healthcare right here in your hometown.

Your contribution, no matter its size, is the helping hand you extend to your neighbor down the street who is undergoing cardiac rehabilitation through DJWH.

 

It is the gift you give to your sister whose breast cancer was caught early because she had a routine mammogram in the DJWH Radiology Suite. 

 

It is the compassion you extend to the young family at your church in need of preventative care that they can secure through the Rural Health Center.

 

In those moments when your heart is touched, you close your eyes and only wish you could do something-anything to help another, know that you can through the Dr. John Warner Hospital Foundation.

 

You Decide

 

Your gift of compassion and caring can be given as an unrestricted donation in which the DJWH Foundation Board and hospital staff work together to determine the best use.

Or your contribution can be earmarked for one of the following hospital services:   Cardiac Rehabilitation, Emergency Department, Laboratory, Medical Surgical Unit, Radiology, Respiratory/Cardiopulmonary Department, Rural Health Center, or Surgery and any other area of your choice.

 

Giving Clubs

 

As a donor, you may qualify for membership in an Annual Giving Club, the Lifetime Giving Club, or as a Legacy Member.  The levels of the Giving Clubs are based on donations given either during the hospital’s fiscal year or over a period of years.

 

Donations made to the DJWH Foundation are tax deductible as allowed by law.  See your tax advisor for more information.

 

ANNUAL GIVING CLUBS

 

Helping hands Club recognizes total annual donations between $50 to $349.

 

Good Samaritan Club recognizes total annual donations between $350 to $699.

 

Foundation Club recognizes total annual donations between $700 to $999.

 

Director’s Club recognizes total annual donations of $1,000 or more.

 

 

LIFETIME GIVING CLUBS

 

Milestone Club recognizes lifetime cumulative donations between $5,000 to $19,999.

 

Bridge Builders Club recognizes lifetime cumulative donations between $20,000 to $34,999.

 

Philanthropist Club recognizes lifetime cumulative donations between $35,000 to $49,999.

 

Distinguished Benefactor Club recognizes lifetime cumulative donations of $50,000 or more.

 

 

LEGACY MEMBERS

 

Legacy members are individuals who have made a commitment for a future gift such as bequests, insurance proceeds, or pledges of $10,000 or more
