Medical Records

Our Medical Records Department is dedicated to providing accurate patient information and healthcare data needed to deliver quality treatment.  The Medical Records Department complies with the HIPAA Privacy standards and will not release you private health information without the proper authorization.  You may obtain copies of your medical records by contacting the Medical Records Department at 935-9571 Ext. 3305.  The Medical Records Department is available Monday-Friday 7:00 a.m. to 4:00 p.m.
